DRAFT pending attorney review. Not final.

Safety Notice

Draft date: October 9, 2026. Version safety-2026-10-09-draft.

You accept this notice when you create an account. It is a draft until an attorney reviews it.

1. Training aid, not medical advice

Perfect Form Athletes comments on movement you film. It does not diagnose injury, clear an athlete to play, or replace a doctor, athletic trainer, or qualified coach.

2. Stop if something hurts

Stop the drill if there is pain, swelling, dizziness, or a movement that feels wrong. Do not use the app to decide that an injury is fine.

3. Check changes with a qualified coach

A score and a drill suggestion are a starting point. Check form changes with a qualified coach, especially for pitching, tumbling, lifting, and any skill that loads the joints.

4. Parents supervise minors

The account holder supervises every minor profile. The adult decides what is filmed and whether a drill is appropriate.

5. Assumption of risk

Sports have inherent risks, including falls, collisions, and overuse, with or without this app. You accept those risks for yourself and, as the account holder, for the athlete profiles you add.

6. Parental release and indemnity for minor profiles

If you add a minor profile, you agree, as a draft term pending counsel, to release PFA LLC and the people who work on the product from claims arising from that athlete's use of the app, to the extent the law allows, and to cover claims brought by or for that minor that come from how the profile was used. A lawyer must confirm whether this sentence is enforceable before launch.
